Coordinated Care in Times of Disaster

Coordinated care networks are challenging endeavors for any community. Developing and launching these networks becomes more challenging in the context of natural disasters and health pandemics, but their value also comes into sharper focus. During this session, you will learn how to leverage data sharing platforms effectively to respond to different types of disasters. You will hear first-hand from colleagues about how they adapted to meet the shifting needs of their communities, as well as difficult lessons learned along the way.
